Teaching NeuroImages: Acute Parinaud syndrome.

چکیده
An 82-year-old man with atrial fibrillation, nonadherent to rivaroxaban, presented with sudden bilateral ptosis. Examination demonstrated bilateral ptosis (figure 1), pupils midline, fixed midsize, not reactive to light but constricting to accommodation, impaired vertical eye movements, paresis of superior greater than inferior rectus, and upgaze convergence nystagmus. MRI showed infarction of bilateral third nerve nuclei and mesial thalami consistent with an artery of Percheron infarct (figure 2), presumed cardioembolic origin. Artery of Percheron is a single P1 branch that supplies the bilateral paramedian thalami and rostral midbrain. Infarcts may present with vertical gaze palsy, memory impairment, and impairment of arousal. AUTHOR CONTRIBUTIONS Emily Swinkin drafted the manuscript, edited images for the publication, and revised the manuscript. Esther Bui conceptualized the study, obtained images for the manuscript, and revised the manuscript for intellectual content.

Teaching NeuroImages: dorsal midbrain (Parinaud) syndrome with corectopia.
A 13-year-old boy presented with emesis, blurred vision, headache, and vertigo. He showed signs of a dorsal midbrain (Parinaud) syndrome (supranuclear upgaze palsy, convergence retraction nystagmus, Collier sign [lid retraction], pupillary light-near-dissociation)1 and eccentric displacement of the pupils (corectopia; figure 1 and figure e-1 on the Neurology Web site at www.neurology.org). متن کاملParinaud Syndrome in Association with Thalamic Infarct in a Young Person
Aim: We report a case of Parinaud syndrome secondary to an acute left posteromedial thalamic infarct in a young person. Presentation of Case: Patient's main presenting complaints were blurring of vision and diplopia. The main clinical manifestations were that of a severe restriction in upward gaze, and convergence-retraction nystagmus. Discussion: Vertical gaze palsies have been well associated...

متن کاملTeaching neuroimages: infant with glutaric aciduria type 1 presenting with infantile spasms and hypsarrhythmia.
A 7-month-old boy with glutaric aciduria type 1 (GA1) presented with 1 week of clustered flexor spasms. Examination revealed mild axial hypotonia without encephalopathy. Video-EEG monitoring revealed hypsarrhythmia and infantile spasms (figure, A). MRI showed acute basal ganglia injury (figure, B). After 3 weeks of prednisolone treatment, 5-month follow-up showed continued resolution of hypsarr...
متن کاملMultiple sclerosis presenting as Parinaud syndrome.
The authors report a patient presenting with a 1-week history of paresis of upward gaze as his initial manifestation of demyelinating disease. They stress that: 1) multiple sclerosis can present as Parinaud syndrome (paralysis of upward gaze); and 2) it is important to rule out a non-contour-deforming intraaxial midbrain lesion if a posterior third ventricle or pineal region mass is not identif...
